Get Fit & Have Fun Liability Waiver:

 

I hereby release, discharge, and hold harmless The Lindsay Lounge Inc., its employees, agents, and representatives, from all liabilities, claims, actions, damages, costs, or expenses of any kind. I voluntarily waive any claim I may have against The Lindsay Lounge Inc. for injuries, damages, or illnesses I may sustain as a result of my participation in any Get Fit and Have Fun programs with The Lindsay Lounge Inc. Such waiver is binding upon the heirs, assigns, successors, and legal representatives whom I may have. 

 

• I confirm that I am physically and psychologically fit to participate in this Get Fit and Have Fun class.
   

• I understand that participating in any Get Fit and Have Fun class includes risk and danger, injuries,and/or death.
   

• I confirm that I will take full responsibility for the risks and make sure that I will not blame the organizers of the Get Fit and Have Fun class.
   

• I confirm that Get Fit and Have Fun class activities require physical strength and involve risks that can aggravate existing injuries.
   

• I understand that I should consult with my physician before joining this Get Fit and Have Fun class.
   

• I release, waive, and indemnify The Lindsay Lounge  including the employees, officers, owners, from any accidents, injuries, damages, or death for participating in this Get Fit and Have Fun class.
   

• I am at the legal age which is 18 years old or older and I am mentally capable of signing this waiver.
